The form is limited to providing information such as your name, email address, and phone number. The privacy policy may also pertain to the establishment of commercial contracts, the use of services provided, or other correspondence involving the exchange of personal data necessary for service provision.

**USERS OF THE FORM**  The data provided in the service availability inquiry form is processed to send commercial information, i.e., to prepare a personalized offer for the services provided by MBVBUS Spółka z ograniczoną odpowiedzialnością. The legal basis for processing personal data is Article 6(1)(a) of the GDPR and your consent to receive commercial information electronically or via telephone devices under the provisions of the Act of July 18, 2002, on Providing Services by Electronic Means. For pursuing claims against the Administrator and for the Administrator's claims and defense against claims – the legal basis for processing personal data is Article 6(1)(f) of the GDPR, where the legitimate interest of the Administrator and yourself arises from other legal provisions (including the Civil Code).  

For sending commercial and marketing information (if additional consent is given) – the legal basis for processing personal data is Article 6(1)(a) of the GDPR. Providing your personal data is voluntary. If you do not provide your data, you will not be able to receive commercial and marketing information.  

Depending on the legal basis for processing, you have the right to access your data, rectify it, object to it, or request its deletion. Under applicable personal data protection laws, you have the right to withdraw consent at any time without affecting the lawfulness of processing based on consent before its withdrawal. Your personal data may be disclosed to:  - Employees or collaborators of the Administrator and individuals comprising the entities operating MBVBUS Spółka z ograniczoną odpowiedzialnością – the Administrator informs that procedures have been implemented to allow access to personal data only to those individuals who have been granted written authorizations and only to the extent necessary for the tasks they perform, and they are obliged to maintain confidentiality of personal data even after the termination of employment or collaboration;  - Your personal data will not be processed in an automated manner and will not be subject to profiling.  

You have the right to lodge a complaint with the President of the Personal Data Protection Office, located at ul. Stawki 2 in Warsaw.
